⚠ Uncontrolled hypertension — flagged 2025, unresolved Carotid Doppler referral (Apr–May 2025, Dr Ajay Chanana) cites uncontrolled hypertension as the reason. No home BP log since Feb 2021 and no repeat lipid or carotid result on file since. Worth pulling forward — a recheck and a current lipid panel would settle whether anything needs treating, especially given Guy's recent three-vessel disease.

Last lipid panel on file: 2004 (normal — TC 4.8, TG 0.9, LDL 3.15). BP log: 2 readings, 2021. Carotid Doppler ordered 2025 for uncontrolled hypertension — result not in this folder.
Explain this to me
Everything on file predates any recent monitoring. A 20+ year-old lipid panel and a 2021 blood pressure reading are not enough to characterise current cardiovascular risk, particularly with a first-degree relative (Guy) now confirmed with three-vessel coronary disease. A current lipid panel, HbA1c, and a run of home BP readings would close this gap quickly.
Musculoskeletal — Left Knee Stable / old injury
2015–17 workup for bilateral medial knee pain. MRI (2016) showed a probable old left MCL injury, otherwise normal. Discharged from orthopaedics in 2017 after a missed follow-up — no evidence of ongoing active pathology.
Immunisation status Up to date
MMR caught up 2022 (was non-immune to measles). Hep B surface antibody 409 IU/L (2012) — well protective. Full childhood + adult series otherwise on file.
