Registering with the Conseil départemental de Vaucluse de l'Ordre des Médecins (CDOM 84), under loi n° 2026-373 du 15 mai 2026.
Began training before 31 December 2020, so Philip's 2005 Southampton qualification is within scope of the new law. Recognition of the specific UK diploma still depends on a ministerial list CDOM 84 hadn't confirmed as published. Below: every step in order, each with a clear action — email, post, look something up, or just do it — and a ready-to-send draft letter where one's needed.
STEP_DATA) — flip a step's status to "done" and re-upload to le-tst and everyone sees the change. The checkboxes below also work locally in your own browser as a personal tracker, but those don't sync between devices or to anyone else — only editing the file does that.
Tap a letter to open it, copy it, and adapt the bracketed [placeholders] before sending.
All four CDOM 84 forms are pre-filled where the information was known and sent to Philip directly as PDFs (this page can't carry the files themselves).
| Form | What's filled | Still to do |
|---|---|---|
| Questionnaire médecin actif | Name, sex, nationality, diploma details, languages, email, department (Vaucluse), personal address (Sablet), situation familiale — married, Danielle's profession, the girls' birth dates (optional section) | Tick the sworn "Non" declarations (Q13–16, 17, 19, 26–29); sign and date page 9; attach a photo |
| Déclaration sur l'honneur | Department (Vaucluse), the four acknowledgement boxes | Add full name on the first line; sign and date |
| Serment médical | Name on the « Serment prononcé par le Docteur » line | Signed in person at the Ordre once the file is accepted — leave date/signature blank until then |
| Fiche consentement casier judiciaire | Name, date and place of birth | Sign and date at the bottom |
A French-language CV for the dossier has been drafted as a separate .docx from Philip's English draft (CDOM 84's document list requires French). It condenses the original and drops the "outside work" section as not relevant to a registration file — Philip checks the dates and figures before sending.
